About Tendonitis
Tendons connect muscles to bones and help control movement, stability, push-off, balance, and foot position. Tendonitis means a tendon is irritated or inflamed, often after overload, repetitive strain, improper footwear, sudden activity changes, or injury.
In the foot and ankle, tendonitis may affect the Achilles tendon, peroneal tendons, posterior tibial tendon, extensor tendons, or other tendon structures. Symptoms can start as mild soreness and become more limiting if the tendon keeps being overloaded.
Pain, swelling, warmth, or tenderness may develop around the affected tendon.
Walking, stairs, running, standing, or rising onto the toes may become painful.
Symptoms may follow repetitive strain, sports activity, poor support, or sudden trauma.

Common Symptoms

Types of Tendonitis

Achilles and peroneal tendonitis are common, but tendon irritation can also affect the posterior tibial tendon, extensor tendons, flexor tendons, and other stabilizing structures.
Achilles tendonitis often causes pain behind the heel or lower calf, especially with running, stairs, jumping, or rising onto the toes.
Peroneal tendonitis usually causes pain and swelling along the outside of the ankle and may worsen on uneven ground or during sports.
Posterior tibial tendon irritation can cause pain along the inside of the ankle and arch and may be associated with flatfoot changes.
Extensor tendonitis may cause pain across the top of the foot, often aggravated by tight shoes, laces, or repetitive uphill activity.

Diagnosis starts with locating the painful tendon, understanding which movements trigger symptoms, and checking whether the tendon is inflamed, strained, weakened, or possibly torn.

Depending on the tendon and severity, care may include activity modification, supportive shoes, orthotics, bracing, immobilization in a boot, medication guidance, physical therapy, stretching, strengthening, or injections in selected cases.
Reducing painful activity gives the tendon a chance to recover instead of repeatedly aggravating the same tissue.
Orthotics, heel lifts, braces, or a walking boot may be used when mechanics or tendon strain need extra support.
