A Heel Spur May Be Part of a Larger Heel Pain Problem.

A heel spur is a bony projection that forms on the heel bone, often where soft tissue attaches to the calcaneus. Some heel spurs appear under the heel near the plantar fascia. Others form at the back of the heel near the Achilles tendon attachment.

Where Heel Spurs Usually Form

Heel spurs may form on the underside of the heel bone near the plantar fascia attachment or at the back of the heel near the Achilles tendon insertion. The location helps explain whether pain is more likely with first steps, shoe pressure, running, stairs, or push-off.

Pain under the heel is often evaluated differently than pain at the back of the heel. That is why diagnosis should look at both the spur location and the surrounding soft tissues.

Plantar heel spurs are commonly associated with plantar fascia stress under the heel. Posterior heel spurs may be associated with Achilles tendon insertion irritation, shoe rubbing, or a bony prominence at the back of the heel.
